Submission Requirements
A 500 to 1,000 word essay that describes the initiative/work of an individual or organization, and how the initiative/work demonstrates the direct link to the MITSS mission of Supporting Healing and Restoring Hope to patients, families, and clinicians following adverse medical events will be reviewed by the Selection Committee. Factors that will be taken into account about the work are:
· Alignment with the MITSS mission. Is the applicant:
o Supporting patients, families and/or clinicians following adverse medical events?
o Respectful of both groups (patients/families as well as clinicians)?
o Including effective partnering?
· Goals (What do you hope to achieve?)
· Impact/Benefit of work
· Innovations and Creativity (How is the work being accomplished?)
· Measures of Success/Ability to effect Positive change (quantitative, qualitative, and/or anecdotal evidence?)
· Efforts to document and track the above results
· Replicability (Possibility of implementing program/work elsewhere?)

Timeline
The deadline for nominations is Friday, September 16, 2011. Applications received after this date will not be considered. The award will be presented at the MITSS 10th Annual Dinner and Fundraiser which will be held on Tuesday, November 1st, 2011, at the Westin Copley Place, Boston, MA.
